According to a press release from People for the American Way, there are several different sorts of voting problems that have already occurred:

Voter intimidation:
California, Maryland, Illinois, New York, Texas

Machines and technology snafus:
Maryland, California, Illinois, Tennessee

New voter ID laws:
Ohio, Arizona, Indiana

A run on absentee ballots:
Pennsylvania, California, Maryland
