The Health Care Industry's PR Scam: Will Obama Fall for It?

In a much-anticipated statement today, Barack Obama announced what is largely a public relations end-run by the health care industry, designed to trim a few scraps off of the nation’s porcine health care budget, while preserving its basic system of medicine for profit.

In a letter to Obama that was released over the weekend, executives from the Advanced Medical Technology Association (the medical device manufacturers lobbying group), the American Hospital Association, the American Medical Association, America’s Health Insurance Plans, and the Pharmaceutical Research and Manufacturers of America, as well as the Service Employees International Union, pledged to “do our part” to reduce health care costs. Their vague, pie-in-the sky promise amounts to just a 1.5 percent reduction in the growth rate of health care spending. Such is the explosion in health care costs that even this miniscule reduction represents a potential $2 trillion saving over 10 years. But there’s no guarantee this figure will be achieved. As the Washington Post points out:

The groups did not spell out yesterday how they plan to reach such a target, and…they offer only a broad pledge, not an outright commitment….In addition, White House officials said, there is no mechanism to ensure that the groups live up to their offer, only the implicit threat of public embarrassment.

“Public embarrassment”? From Big Pharma and the health insurance companies–-two of the most shameless industries in the history of corporate capitalism? In any case, even if the $2 trillion reduction is achieved, it clearly won’t come out of industry profits. The Post reports:

Signers of the letter said that large amounts could be saved by aggressive efforts to prevent obesity, coordinate care, manage chronic illnesses and curtail unnecessary tests and procedures; by standardizing insurance claim forms; and by increasing the use of information technology, like electronic medical records.

So let’s get this straight: Saving all this money depends on getting Americans to eat less? Good luck with that one. And the other brilliant cost-saving measures involve getting doctors to create computer records of all the overpriced drugs they prescribe, and giving patients easier forms to fill out before they get turned down six times by their private insurance companies?

Do you see a pattern here? None of these changes would make a dent in the industry’s bottom line--and what’s more, they could even enhance profits, by encouraging government-funded programs to help private companies streamline their bloated bureaucracy (much of which would instantly become superfluous under a public, single-payer system). The letter to Obama suggested this when it said: “We are committed to taking action in private-public partnership to create a more stable and sustainable health care system.” We all know by now that “private-public partnership” usually means public investment for private profit.

It all adds up to a brilliant move, when you think about it. It makes the private health care companies look cooperative and proactive, rather than like the greedy obstructionists they really are. It gets these companies on the inside track with the administration, and creates common cause with the unions. In particular, it establishes a solid place at the table for the health insurance industry, the blood-sucking middlemen who ought to be kicked out of the health care system altogether. 

And what might the industry get in return for this generous “cooperation”? The Kaiser Daily Health Policy report today rounded up the possibilities:

The [Wall Street] Journal reports that although the groups did not ask for anything in return for the pledge, many of the factions are looking to prevent regulations that could “pose new burdens” or affect their profitability. For example, the health insurance industry is seeking to offset any reductions to their payments by obtaining new rules that would require all U.S. residents to have health coverage, according to the Journal. The Journal reports that health insurers have made several concessions intended to prevent a public option — which they fear could affect their profitability — as part of reform legislation (Wall Street Journal, 5/11). According to the AP/Philadelphia Inquirer, drugmakers are hoping to avoid a requirement that new drugs pass a cost-benefit test before receiving regulatory approval. In addition, hospitals and physicians are looking to avoid a system in which the government would dictate their payments for all patients, not just those under Medicare or Medicaid (Alonso-Zaldivar, AP/Philadelphia Inquirer, 5/11).

In other words, the underlying purpose of this PR stunt is to slow or block any meaningful health care reforms, which could actually improve care while reducing the price tag by a lot more than 1.5 percent. These include regulating the cost of pharmaceuticals and medical devices, curtailing or eliminating the role of the insurance companies, or introducing single-payer, which allows other developed countries to deliver superior health care for 20 to 40 percent less--all of which make $2 trillion in weight-loss programs and paperwork reduction measures look pretty pitiful by comparison.

All we can hope for is the possibility, remote as it may be, that Obama himself is also playing a PR game--making nice with the industry shills while planning some kind of genuine reform that will hit them in the only place that counts, and the only place where truly meaningful savings reside: their profit margins.

I'm not so sure I would character Obama as the one needing warned not to fall for big-health care's "scam." It's American's who will continue to suffer from a health care system in crisis. The health care industry isn't trying to pull the wool over Obama's eyes; Obama and health care are trying to scam the American people. I don't understand why the dots aren't being connected by the media?

Hearing what you want to hear

The MSM has learned a lesson about telling people what they should know. Don't! People only want to hear news that agrees with their perspective of the world "they" live in; reality sux! People really do believe what they hear on the TV, radio and read in their favorite newspapers that cater to their perspective. So if their world says Obama's universal health care is bad ... it's bad, no more questions. Besides, no one ever thinks they'll be in an auto accident where they will be out of work for a year or two, with bills to pay mounting up and no income - it always happens to someone else. Besides, they have a nest egg set aside for such occasions - they just never understand just how expensive things are when you have a limited income and no way to replenish cash once its spent.

I know this because I have lived thru such a nightmare - it's not fun and you never get back to the perch you had before you were knocked down.

Exercise in Jerking Off

Unless Obama puts universal health care/single payer at the top of the food chain in this debate we're all just whacking our puds for no useful purpose. The insurance industry's sole purpose is not to provide the best medical care for its insured, it's to provide the highest shareholder equity possible by limiting the number and amount of payments for services used by their insured. If an insured is stricken with an ailment that would lower the profit margin, thereby reducing shareholder equity, efforts will be made to drop the insured as soon as all legal challenges are dealt with. That's definitely not in the best interest of the insured; it is for the shareholder. So is Obama and his administration going to support the people/public/ insured or the industry/profit/shareholder?

Obama is the perfect health industry spokesman

There are plenty of places to put blame for why we aren't going to get single payer in my lifetime. I think I'd like to top my list with the basic fact that we have achieved a de facto fascist government in the classical meaning of the term. The two political parties have re-written the rulebook to make sure the exclude outsiders from any part of the decision process. Only corporations have real access to the legislature; there is a revolving door between corporations and appointed/elected officials to ensure that the process is corrupted as it can be and rotten to the core.

It should come as no surprise that Obama is not in favor of single payer. I have a particular interest in this issue and as I watched the candidate carefully as a precinct captain in Iowa, I watched his message change. Mostly it was ambiguous. After he won Iowa, he was done with us and threw us under the bus. I know a lot of people who worked on the campaign here; this was a campaign of progressives, college students, union activists, and professional slackers (like me). Obama started running right after Iowa, and since Iowa has a huge health insurance industry it's no surprise that he didn't want to alarm his big donors. So he started making noise on the campaign trail about "affordable insurance", which means "no single payer, the same old same old you've got right now".

I dumped the Obama campaign, as it had dumped me. I worked for Nader after that, one of the people who got him on the ballot here. But again we got snookered by the binary system. People are afraid to REALLY vote for change, but they can vote for somebody who is lying about changing things.

FACE IT... our political system is fucked up. It is infiltrated and intertwined and integrated with the interests of business and those are NOT your interests. Unless you're one of the CEOs or other top 1 per centers who are living large off of your tax dollars and laughing at you sweating down in the filthy pit while you try to hang on to your health insurance, your life's savings, your retirement plan, your house, your marriage... I think they must get a charge out of human misery. They can't cause enough of it by stealing everything that's not nailed down so they start wars all over the world so they can blow women and children up and torture with impunity.

Well, I got off topic on health care "reform". Sorry. But watch carefully, because when it finally is "reformed", what will happen is that your tax dollars will be used to maintain the profitability of the current system. I PROMISE that.

The Trouble with Capitalism

The way I understand it the officers and directors of any publicly traded company have a fiduciary duty to maximize profit. Shareholders could sue - and win - if Big Pharma and Big Insurance agree to any plan that reduces their profits. I don't see any way to reconcile the phrase "for profit health insurance".

Is someone giving ut thier cheese?

At most Pharma's and the insurance companies pledge is not to raise prices on existing products more than 2% a year greater than inflation. Sounds more like an income guarantee to me.

The plan for them is that the government increases their markets by 30 percent by increasing the number of insured people - the Corpotations get fat and happy.
They get a locked in 2% increase (perhaps above inflation) not bad when drug prices usually are fairly stable after going on the market.
They don't have to lower their prices for new drugs or new devices which will come to the market later.
There is no enforcement.

I work in the medical device industry. I was a practicing physician. I can't see where the corporations are giving anything away here. My company did not agree to anything, we did not sign any agreement. Yes I get and read the industry journals. But they device industry association has zero control over what my company does. The industry has given up nothing and gotten a PR boost.

What this does is gives an opportunity now for the poor itty bitty companies to claim how unfair this is and how they need lower taxes to survive.

I'm disappointed. My company will make money in a competitive environment when we produce better products that help deliver better health and which save cost by getting people better faster.

Better health is the key. If you think we have an energy crisis - bigger is the health crisis. It consumes twice the money. Most heart disease, obesity and diabetes are lifestyle diseases. A lot of cancer is too, and even more could be prevented. Childhood asthma is epidemic in the cities because of pollution.

Medications and hospitals cannot give health - they only treat disease. Treating disease cost at least $30,000 for each year of life saved. Health is cheap in comparison. We can not drill our way out of the energy crisis. There are very few curable diseases, thus we cannot treat our way out of medical cost problem. The "green" solution is health promotion. It can be done - but Pharma and the insurance companies are not going to promote it any more than Exxon promote carbon reduction.

Well, Mr Ridgeway, is the question really whether Obama falls for the PR scam by Big Pharma and the private underwriters? Or, is it whether Obama is part of the PR scam by the "industry"? If Obama is somehow "snaking" the "industry", seducing them, or just a naive Savior in Chief who will suddenly wake up and turn on the evil corporatists, then why has the White House so completely blacked out media coverage of a national public guaranteed access to disease and trauma care system, a medicare for all like option, oft called "single-payer"? Why has Congress refused to even allow representation and open discussion of such an option? Why are professional providers of care who support this option physically removed and arrested amid arrogant sneering by US Senators when simply asking for a place at the table? Why is the Savior in Chief and Congress maintaining the illusion that "coverage" equals "health"? Its time to take off the tea shirts. put down the memorabilia and campaign buttons, and start addressing governance and the restoration of the republic, perhaps even a modicum of democracy. The Great Manipulator

Obama campaigned on bringing change to Washington, but as it turns out the Shadow Government in Washington, represented by corporate interests, has converted him into the Great Manipulator, a proud member of the greatest democracy money can buy. Obama talks about saving money with a new form of universal health coverage - - a far cry from the more frugal Single Payer concept - - but what he really means is that the health care industry will make more money while the quality of coverage will continue to decline over future decades. Profits will continue to be more important than health care delivery. I have never been more disappointed in any President, other than Bush, in my lifetime. Turns out Obama is nothing more than fancy icing on a cardboard cake. Once you cut into his persona there is no substance other than a well decorated public image.

Fool Me Once....

The Big Pharma and the insurance companies have been scamming the country for many years. Consequently, the American health care system is vastly inferior to, say, the French - and vastly more expensive. The insurance companies are not going to lose easy revenue lying down, that's for sure.

Obama has let us down...

No, Obama hasn't changed his opinion, just his support for single payer. Thank the insurance industry's $46 million in campaign contributions in 2008 for that. Aren't political bribes nice?

It never ceases to amaze me, the amount of energy that can go into a project just to avoid doing the right thing. The best, simplest, least costly, most effective thing we could do is expand what has been working so well for years, Medicare. You get sick, you get care, and the caregiver gets paid. Nothing could be simpler.

As well, a single-payer system would bail out virtually 100% of corporations, not just a few banker friends of Geithner. It would save every corporation $6500 per employee per year, compared to Canada's $800, and these companies could spend that money on US salaries and not have to outsource to other countries that already have their universal health care. Yea, we taxpayers would pay, but we'd save $400 million per year in the process.

Stick'em Up: Your health care or your home

Health Care - Live or Die

If you have no health insurance in the U.S. and you need health care you go to a doctor/hospital, get billed after your procedure and then have to pay a lot of money out of pocket. If you don't have that money you go into debt or you file for bankruptcy and/or you lose your home because you can't afford to pay for your health care.

People who have health care insurance whether it's private or public don't know or don't care about the fact that you have to make this decision to sacrifice your home and financial health if you have a big health care bill.

They believe the bill will be taken care of by their insurance. Imagine how many people go bankrupt as a result of an illness - their own or their parents illnesses.

I would venture a guess that if you're not rich or highly insured you know someone who has only had this one choice - get the health care they need and go bankrupt or don't get the care and live in poor health or die.

With 100's of thousands of people losing their jobs more people will be faced with this dilemma in the coming years - get health care by sacrificing their homes or die, health or home, live or die.

America offers very sad options for their citizens. It's a sad time to be an American.

Who wants their medical records online for all to see? Not me!!! That's going to make our records unsafe. Get the insurance companies and Big Pharma out of the decisions. They only have their best interest at heart, which is making more money. Health care should not be a business. The Medicare plan is working just fine. Yea, that is why they raised my small business group plan twenty percent this year, making it almost unaffordable.

The single payer needs to be back on the table. I know too many people in middle American who are struggling just to pay their premiums and getting poor care anyway.

If we want to acquire a better health care system, now is the time to use a computerized medical record system. Computerized medical records is a growing industry, probably soon to put a lot more people to work. Computerized medical records allow for physicians to link up and network in many ways, including no cross prescribing of medicines, which that will make it harder for addicts to keep popping pills. It can link up pharmacies with clinics so they know who has what prescribed for them. It's worth an installment loan for hospitals and clinics to get their records online, but secure – you can't twitter this kind of stuff. The electronic transcriptions business is going to grow over the next decade, and a good job in computerized medical records could mean some debt relief.

Given the severity of this unprecedented crisis since the great depression in 1930s and the social inequalities in the United States, a well thought paradigm and a method of practice intelligently consolidated are strongly needed to improve things and end the recession as quickly as possible at the beginning of the next decade.

The Gini coefficient is larger in the US than any other OECD countries. This resulted in what we saw on August 28, 2005 when Hurricane Katrina hit the southern side. By seeing images on French TVs, I personally thought that the catastrophe was happening in one of the poorest countries in the world. I stopped dreaming when I heard the reporter naming the United States. So giving the opportunity to each American for instance the Obama’s healthcare plan and opportunities to afford tuition for college degrees to those who have the ability to achieve the educational goals will maintain the nation’s legacy and its supremacy. And that will make Obama a great president, and give him the tools he needs to defend the United States position in the world, not only militarily but economically and socially, too.

When President Obama is trying to reach out this goal, political battles rise in the senate and republicans will never give him a gift. And yet political opposition should not be an obstacle to tackle these issues at this time. But As President Obama has promised a constructive plan to any major challenges the country faces, and his impressive collective approach to address major concerns, plus his abilities to take responsive actions, I believe that he will make it.

The proposed cost savings

The proposed cost savings methods they outlined do seem a little foolish at first glance... however.. a HUGE portion of the nations health costs are directly attributed to obesity. Overweight persons are MUCH more likely to develop type II diabetes which is arguably the single largest financial strain on the health-care system. If americans want low-cost universal health care to be a reality its time we take a little responsibility for our actions. Put down the cheeseburger and hit the treadmill. Want healthcare to cost less for the masses? Then we as a country need to stop being so unhealthy. Just my two cents.

Healthcare issue is the nightmare of our society

I think Bush didn't anything to fix the healthcare industry in our country... either did Clinton... either will do Obama.

Doctors sucks us every time we go to a hospital the same way doctors are sucked from insurance companies when buying policies, the same way they are sucked at gas pump stations, the same way they are sucked from mortgages and credit cards giants.... etc, etc.
